Registered Nurse (RN) - Home Based Care
Mountain Regional Hospice and Rockbridge Area Hospice have merged to create ConnectionsPlus Healthcare + Hospice. We are a well-respected non-profit service organization, with a focus on family and community support. We are expanding our serious illness and end-of-life services and have an opening for a full-time registered nurse (RN). The RN is part of an interdisciplinary team focused on providing quality care to patients and their families in their own homes or residence.
The ideal candidate has a compassionate character and a willingness to be present and comforting for people dealing with life transitions. An inquisitive mind, good communication skills and flexibility are a must. Candidates must possess a current Virginia RN license. Two least two years’ work experience in Home Health and/or Hospice experience preferred, but we will train nurses who have the right heart for this type of care.

Social Worker/Counselor
Rockbridge Area Hospice/Connections+ is looking for a social worker/counselor to join our Integrated healthcare team, with a focus on Community Based Palliative Care. This position will work with individuals, families, and community groups, providing support and education as they deal with complicated chronic illness. Issues addressed include end-of-life planning, caregiving, anxiety and depression, and grief.
Our ideal candidate is comfortable visiting people in their homes, has experience in a holistic approach to healthcare - addressing the inter-relationship between medical illness and emotional well-being. A minimum of a Bachelors degree in social work or counseling is required; License is preferred. Position is full time w/ excellent benefits. Part time is a possibility for the right candidate. COVID19 Vaccination Required.
